Software quality assurance state of the art and best practices?

On HN we often see articles about the state of the art and best practices in software engineering. Out of curiosity, what resources are available for learning about the same for software quality assurance?

Most of us don't think about it, but despite our best efforts there will probably be some bugs, so what is the best way of finding them, documenting them, etc. all the way from manual testing to automated testing?

What are the most popular tools? (For example, I have heard of TestLodge and FogBugz, but do these cover all the bases?)

Where are there resources about how SQA should be done? Is there an equivalent in SQA for all the newsletters, video courses, etc. that exist for software engineering?

Here's another angle on the question: is it possible to do SQA incorrectly or badly, assuming you do it at all?
