1- most probably, money, buy ANI service, abuse it pretend you are calling from number you are not. Bolster it with website&service abuse

2- probably fixed, but a callID transmitter chip could be used to exploit a lengthy writable time for the data. it became difficult to buythese chips, and with voip & sip things are very different now. you may see this in low/old service areas,but its probably extinct.

3- voip abuse, part of voip is setting up caller number, some allow this to be changed on some basis,pay to change, make a fleet of numbers, some services exist soley to spoof the caller number.
